Guidelines
1. You must present a photo ID on the first day of class.
2. If you are late to class, you will not be admitted. This includes returning late from breaks.
3. There are no refunds for no shows. A 48-hour notification is required if you cannot attend. Email abrinson@elitenursingservices.org or contact Angelia Brinson at 330.634.5633. Your payment will be credited to use within 30 days.
4. In the event the nurse trainer must cancel class you will be contacted immediately, and the class will be rescheduled. If the rescheduled class does not meet your availability, a refund will be issued.
5. American Disabilities Statement: Students with documented disabilities who may need special instructional accommodations must notify instructor and agency at the time of registration.
6. Non-participation, distracting behavior, monopolizing conversations, or rudeness to instructor or other students will not be tolerated and may result in permanent dismissal from the class.
7. The Certificate of Completion will be sent by DODD to the email address you enter on page 2 of the application.
8. The temperature in the training room fluctuates. I recommend that you dress
in layers.
9. Smoking is not permitted in or near the building.
